Root Cause Analysis Courses for Healthcare Professionals: The objective of root cause analysis (RCA) is to find out what happened, why did it happen and what you need to do to prevent it from happening again.

Originally used in manufacturing, the RCA approach to problem-solving has been especially embraced by the healthcare industry.

A central tenet of RCA is to identify underlying problems that increase the likelihood of errors while avoiding the trap of focusing on mistakes by individuals.

In healthcare this translates to using RCA’s systems approach to identify both active errors – those occurring at the point of interface between humans and a complex system – and latent errors, which can be viewed as hidden problems within healthcare systems that contribute to adverse events.

RCA is one of the most widely used retrospective methods for detecting safety hazards.

A classic real-life example of RCA/healthcare in action is the case of a patient who mistakenly underwent a cardiac procedure intended for another, similarly named patient. The traditional analysis method would be to find blame with specific employees responsible for the grave and likely litigious error.

However, RCA was deployed to get to the underlying causes of this issue. Subsequently, the RCA methodology revealed 17 distinct errors. One of the most important revelations turned up an organizational factor showing how the cardiology department used an error-prone scheduling system that identified patients by name rather than by their medical record numbers.

The hospital’s RCA team also discovered an environmental error factor: A neurosurgery resident who suspected the mistake did not challenge the cardiologists because the procedure was at a technically delicate juncture.

Bottom line, the root cause analysis approach led the hospital to implement important systematic changes to reduce the likelihood of similar errors in the future.
