One of the most important revelations turned up an organizational factor showing how the cardiology department used an error-prone scheduling system that identified patients by name rather than by their medical record numbers.

The hospital’s RCA team also discovered an environmental error factor: A neurosurgery resident who suspected the mistake did not challenge the cardiologists because the procedure was at a technically delicate juncture.

Bottom line, the root cause analysis approach led the hospital to implement important systematic changes to reduce the likelihood of similar errors in the future.
